All new patients will be asked to fill out a lifestyle questionnaire and to see the
practice nurse for a brief health check. They may also be asked to see one of the
doctors in surgery time.

Please let
us know if you change your address so that we can keep our records up to date, and
advise you whether or not your new address is still within our practice area.
It obviously helps us, too, if we have your current telephone number on file.
